FEES AND INSURANCE



Insurance

Currently Dr. Gamble accepts Managed Health Network, Molina, Magellan and Beacon Health Options as an in-network provider. For all other insurance plans, Dr. Gamble can provide a superbill to allow you to get reimbursed from insurance as an out-of-network provider. Please contact your insurance provider before your appointment to understand your benefits – including co-payment, co-insurance and deductibles. Additionally not all services provided by Dr. Gamble are covered by insurance, these include but are not limited to premarital counseling, some family counseling services and psychoeducational assessment.

Dr. Gamble fee for services is $135 for the initial intake session and $125 for 45-52 min sessions. Couples and families and some individual sessions require longer services and sessions 53 minutes or more are billed at a rate of $135. Psychological testing is billed at a rate of $150 and hour, but Dr. Gamble will discuss with you in the initial evaluation the estimated cost of a the psychological or psychoeducational assessment needed.

Sliding Scale

Dr. Gamble offers a sliding scale for a certain number of clients and has a student rate for client completing required psychotherapy hours for completion of their graduate programs. Please contact the office to discuss these criteria for eligibility for a sliding scale rate.



Payment

Dr Gamble accepts Cash, Check, Credit Cards and most HSA Cards as payment for services, co-payments, co-insurance and deductibles.

No Surprises Act


Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
